    Private Slovak in WWI for desertion I know of no executions in the US for insubordination.

    In Greece or Turkey however an Officer has the right to shoot an insubordinate soldier on the spot.

    The last one I can find is John A. Bennett, who was executed on 13 April 1961 after being sentenced on 8 February 1955. The charge was rape and premeditated attempted murder.

    After him, I can find no mention of any serviceman being executed under the Uniform Code of Military Justice (UCMJ), although the UCMJ (to my knowledge) still permits execution for certain crimes.

    EDIT: There are currently nine people on Death Row at the USDB in Fort Leavenworth, so the US Military does still assign the death sentence for certain crimes, just that executions are very, very, very infrequent (since an execution requires Presidential approval). Although the last execution was John A Bennett in 1961.
