Should I ask my manager before using them as a reference?

I am wondering if it's disrespectful not to ask before putting their name on the application? When I ask her should I do it in person or should I call her? I work at a department store so should I just put the store because I'm guessing they will ask for or should I ask for her personal phone number?

    Yes. You can't use people are reference without letting them know.

    Yes you should tell them, but first ask them if you can use them as a reference or if possible get a reference letter from them. Most people don't like to be surprised by having some one call them up and asking questions about you.

    Just have a chat with your boss and ask them if they can give you a reference letter.
