Does this sound like ovarian cysts, or something else? I am at a loss!?

In November 2011, I had a 10lbs ovarian cyst removed. I was 17 at the time. The symptoms which I had been having (shortness of breath, majorly distended stomach as if I was pregnant, firm abdomen, kdiney issues, etc.) subsided after that, including losing about 20 lbs in total from the water retention letting up from the cyst. I had some major side effects from the surgery, including irregular cycles, some hair loss, hormonal fluctuations, depression and postsurgical fatigue and exhaustion, which then led tu burn-out as I was not only doing my final exams, but also working as a paramedic and as head of the student council. Alas, not a very happy time for being ill.
However, I felt like I had gotten over the brunt of it about a year after the surgery. I felt better, was stronger and was finally at the mental and physical capacity I was before. About four months ago, I started noticing some changes for the worse, and I am worried due to what happened the last time I wasn't feeling well. I am lethargic, lightheaded nearly all the time, I would have acne if I wasn't treating it with some heavy duty acne medication, I have migraines (while I have had them since i was two years old, there has been significant change for the worse in the past few months. Not in their frequency, but in their severity. I have a high pain tolerance, but two of them in the last little while have left me a crying mess in bed for two days. It was not pretty).
I have been gaining weight and losing all my muscle tone, partly because I am too tired to keep up my usual routine (I have cut down from running my usual 30 min 3-4 times a week and extra strength training). I am getting seriously frustrated because while other people my age (I am turning 19 in two months) are out partying or going to college, I am too exhausted to even think about it. I occasionally have severe pain in my abdomen, sometimes just in the lower corners on one side, at others spanning from my lower back to my ribcage. One time, I was in the bus on my way home after walking a lot during the day (when I tend to get bloated and my abdomen starts to hurt) and the pain came in waves so strong I could not stand up and get off at my stop. This only happens very rarely, but I am seriously worried because none of my doctors seem to think there is anything wrong! I have an appointment with my gyno next week, but I am at a loss. I eat very clean, as I am a vegetarian and try to cut down on any processed foods. I estimate I take in about 1500 calories a day. I have steadily gained about 12 lbs over the last few months for absolutely no reason (which was also while i was working out). This was not muscle mass, but body fat which I measured at the gym. I am taking in adequate nutrients, so i really donät know what's wrong with me. Any help would be VERY much appreciated. Sorry for the rant, but I am desperate for answers.
