How to approach a girl I don't know?

ok, im a 16 year old guy (nearly 17) and i have recently started college and i love it! 
And in my tutorial and RE class there is this beautiful girl that i really really like. She has blue eyes, blonde hair, nice body and personality (my dream girl you could say).
I also think that she likes me because I always catch her looking at me, and when we look at each other, she quickly looks away and twiddles her hair.
However, I only see her 2 hours a week, so it is quite hard to get to know her.
I have also never had a proper girlfriend even though I am good looking, because I am quite shy and not that confident with attractive girls, but I really want to change this.
I already kind of have a plan to approach her after class but I'm not sure. Something like this:
"Hey, I don't know you that well, but I was wondering if you would like to hang out after college or during break sometime"
Then if all goes well, swap numbers or something.
It won't be exactly like that of course. I will also chat to her before I ask and act confident.
But do you think this will work? Or will she be scared off because we don't know each other?
I mean, if she likes me (which I am pretty sure of) all should go well right?

It sounds reasonabale to me - but maybe a little direct if the girl doesn't know you that well - it also forces her into a yes/no answer which she might not feel comfortable with and she may well decide on no - just because she doesn't really know you. What you need is an amusing opening line, something funny (not rude) but not corny like "If I said you had a beautiful body would you hold it against me". Whatever it is you need to gert her to say something back to you to start a convversation - even if it is quite short because then you can say "I would love to talk some more - what about hanging out at break, after school, etc." Whatever the approach try not to make it sound rehearsed otherwise you are going to come across like a bit of a stalker. Most importantly just be yourself. I don't know the girl - I don't know you  and there are no guarantees in life but if you don't ask you will never know. Good luck!!
