How are number of deaths caused by secondhand smoke calculated?

I'm not finding information on ETS being a direct cause, or even an indirect contributor to a primary cause (cancer, heart attack, etc.). Are there any statistics on ACTUAL cases? If not, how the heck are number or deaths figured?

There is also a chart they give that shows what causes of death they attributed to the second hand smoke.

You might find the following report useful as well though it is substantially larger than the New Zealand report.

If second hand smoke was as dangerous as people believe it is, there would be nobody alive in their 50s and 60s.
