You really have to see this film more than once to get it, if you think the movie is reality and that all of the dreams were actually Fischer's, then you got the whole movie wrong. The movie actually is completly set in Saito's lingo. If you pay attention to the first sequence of the film, you'll notice that Saito does two things, he uses the same line as Mal "leap of faith" and he is playing with Cobb's totem. Saito convinces Cobb to do the "job" by a "leap of faith." Cobb is chased by a bunch of agents, Saito saves him. Cobb and Saito visit Yusef who brings them into a basement with various figures connected to the dream machine. The idea was for Cobb to experiment with the deep sedative. He does, and when he "wakes up," he tries to use his totem only to be interrupted by Saito. Cobb never does find out if he is in the real world or not. In fact, he hasn’t been yet. He’s been in limbo ever since he got there with Mal. Ever since then, he's been going deeper and deeper to the point where he created Saito as a projection to help him "get back home" - Did you really think Saito can just pick up the phone and make murder charges disappear? No. But, Cobb believes it and thus Saito is used to thrust Cobb further and further into a state of limbo – where at the end of the journey, Cobb truly believes he is with his children after confronting and getting over his projection of Mal. So, yes, the ending is a dream, he is still in the Lingo. But since the Inception was performed sucessfully on him, the top begins to topple, meaning Cobb believes that he is actually home and not the in the lingo. The kids wearing the same clothes etc. is another hint as well that he never left the lingo.
It's much more complex than this, there is a ton of different levels of this film and you have to keep digger deeper. I'm definately going to see this several times on DVD.

I have been thinking about that all morning! I want to think it was real - it sort of looked like the top was losing momentum, but I keep getting stuck on the fact that the children are in the same postions that he kept remembering them in in his dreams. Hard to say for sure, but I think my final answer is: reality. The other team members came back to reality and he was there with them.
